Form Friday: Getting Familiar with OCFS Forms

As a childcare provider in New York City, it’s important to be familiar with the forms from OCFS, the Office of Children and Family Services. This week we take a look at OCFS-LDSS-7006, the Individual Health Care Plan for a Child with Special Health Care Needs.
What is it?
OCFS-LDSS-7006 documents a plan for how to care for a child with special health care needs. The form helps ensure the childcare provider has all the information and instructions they need to adequately care for the child.
When do I use it?
A child with a special health care need is defined as a child who has a chronic physical, developmental, behavioral, or emotional condition expected to last 12 months or more and who requires services beyond that required by children generally.
Children who may have special health care needs are those with allergies, disabilities, or medical conditions. They may require services such as early intervention, speech therapy, or the use of epinephrine auto-injectors.
How do I use it?
The child care provider completes OCFS-LDSS-7006 working with the child’s parent and the child’s health care provider. Once completed, the form requires the signature of the child care provider and the parent.
According to New York State Child Care Regulations, the child care program must maintain children’s individual healthcare plans on file at the family day care home, available for inspection by OCFS or its representatives at any time.
