Form Friday: Getting Familiar with OCFS Forms

As a child care provider in New York City, it’s important to be familiar with the forms from OCFS, the Office of Children and Family services. This week we take a look at OCFS-LDSS-7002 Medication Consent Form
What is it?
OCFS-LDSS-7002 may be used to meet the requirements for the administration of prescription medications, oral over-the-counter medications, medicated patches, and eye, ear, or nasal drops/sprays.
It’s important to note that only staff certified to administer medications to day care children are permitted to do so.
OCFS-LDSS-7002 collects
instructions for administering a specific medication (time, dose, frequency, etc.) by a licensed authorized prescriber
The parent’s consent for the child care program to administer the the medication as instructed
The child care program’s confirmation that all information needed to administer the medication has been provided
When do I use it?
One form must be completed for each medication.
Multiple medications cannot be listed on one form
Consent forms must be reauthorized at least once every six months for children under 5 years of age and at least once every 12 months for children 5 years of age and older.
How do I use it?
A licensed authorized prescriber completes and signs sections 1 - 18.
The child’s parent completes and signs sections 19 - 23 and, if applicable, 31-32.
The child care program completes and signs sections 24 - 30 and, if applicable, 33-35.
