Form Friday: Getting Familiar with OCFS Forms
OCFS-6013 and OCFS-6020
As a child care provider in New York City, it’s important to be familiar with the forms from OCFS, the Office of Children and Family services. This week we take a look at two forms that go hand in hand: OCFS-6013 Transportation Consent Form and OCFS-6020 Transportation Plan.
OCFS-6013 Transportation Consent Form
NYS Child Care Regulations state a child care provider must obtain written consent from the parent of a child for any transportation provided or arranged by the provider.
To meet this requirement, providers can use the OFCS-6013 Transportation Consent Form.
OCFS-6013 can also be used to inform a parent when there are any changes to the transportation arrangement, such as who is providing the transportation.
A parent or guardian must sign and date OCFS-6013.
OCFS-6020 Transportation Plan
NYS Child Care Regulations also state a child care provider must provide, at the time of enrollment, a copy of the program’s transportation plan to any parent whose child will receive transportation services.
Providers can detail the program’s transportation plan using the OCFS-6020 Transportation Plan.
OCFS-6020 details regulatory requirements regarding transportation, such as who will provide safety seats for children.
OCFS-6020 also details program-specific information such as where the program will display daily transportation schedules.
